The Services that the Company (Mastering Health and Happiness, PLLC), including Company Owner, Amy Stenehjem, M.D., provides are NOT payable/reimbursable by the Client’s health insurance company. The Client must pay the Company’s fees without any expectation of reimbursement of such fees from any third party.
The Company, including Company Owner, Amy Stenehjem, M.D., is NOT providing the Client with professional medical advice of any kind. Services rendered are limited to providing the Client with general health information, logistical support, guidance, and education. The Client should seek medical advice from one or more duly licensed physicians before making any decision related to his/her medical care. The Client acknowledges that any decisions must be made by the Client and/or the Client’s representative, and NOT by the Company, including Company Owner, Amy Stenehjem, M.D.. The Client’s relationship with the Company, including Company Owner, Amy Stenehjem, M.D., does not create any kind of a physician/patient relationship.
The Company, including Company Owner, Amy Stenehjem, M.D., makes every effort to provide the Client with a secure and confidential means of communication through the Company’s Email System and Scheduling/Billing/Chat Portal. The Client is responsible for maintaining the confidentiality of, and accepts responsibility for, all activities on his/her accounts.
